Welcome Letter

Purpose: Your guide to a smooth, successful passport application.

Dear [Customer First Name],

Thank you for placing your trust in American Passport. We have carefully prepared this package to guide you through every step of your U.S. passport application — from confirming your eligibility to submitting your completed documents.

Here is what we have included for you:

  • A personalized Eligibility Check to confirm whether you qualify for renewal (DS-82) or must apply as a new applicant (DS-11)
  • Step-by-step form completion guidance so every field is filled out correctly the first time
  • A Document Checklist to ensure nothing is missing before you submit
  • Clear Submission Instructions including mailing addresses, in-person requirements, and current processing timelines

Your passport photo has been professionally prepared and is enclosed. It meets all U.S. Department of State specifications.

Important Reminders:

  • Do not sign your application form until instructed — signing in the wrong place is one of the most common reasons applications are returned
  • Use a trackable mailing service when submitting by mail and retain your tracking number
  • Processing times are set by the U.S. Department of State — we will be happy to help you interpret any status updates you receive

We wish you safe and successful travels.

The American Passport Team

Document 1C — Lost or Stolen Passport Guidance

Purpose: Protect your identity and get a replacement passport as quickly as possible.

Step 1 — Report It Immediately

You must report your passport lost or stolen to the U.S. Department of State. This cancels the old passport and prevents fraudulent use.

  • Online: travel.state.gov → “Report a Lost or Stolen Passport”
  • By phone: 1-877-487-2778 (Monday–Friday, 8am–10pm ET)
  • By mail: Submit Form DS-64 along with your new DS-11 application

Step 2 — Complete Form DS-64 (Lost or Stolen Report)

☐ Download Form DS-64 at travel.state.gov
☐ Complete all fields accurately
☐ Submit DS-64 together with your new DS-11 application — do not submit separately

Step 3 — Apply for a Replacement with Form DS-11

A lost or stolen passport requires a full new application. You cannot renew a lost or stolen passport.

☐ Complete Form DS-11 (see Document 1B and Document 2, Section B)
☐ Gather all required documents (see Document 3, DS-11 checklist)
☐ Submit in person at an authorized passport acceptance facility
☐ Submit DS-64 at the same time

Step 4 — Request Expedited Processing if Needed

If you have urgent travel within 3–5 weeks, request expedited processing or book an appointment at a regional passport agency.

  • Expedited processing: Add the expedite fee and note your travel date on the application
  • Urgent/emergency appointments: Call 1-877-487-2778 or visit travel.state.gov to book

⚠️ Important Reminders

  • Never leave a lost or stolen passport unreported — it can be used for identity fraud
  • If found after reporting, the old passport is permanently cancelled and cannot be reactivated
  • Keep a photocopy of your passport in a separate location when traveling

Not sure what to do next? Contact us at hello@americanpassport.us — we are here to help.
